Job Description

• The Physiotherapist will work as a part of a multi-disciplinary team, including health and social services colleagues to meet the needs of clients referred to the Reablement Team
• To conduct Physiotherapy assessment for clients receiving input from the Reablement Team
• • To provide Physiotherapy treatment and rehabilitation plan for clients with the Reablement Team ensuring that support workers are able to follow the rehabilitation plan safely and efficiently with clients.
• • To work within the multidisciplinary team as an autonomous practitioner, ensuring the highest possible standards of practice
